Silver Lake Specialized Care Center, a leader in rehabilitation, provides a comprehensive rehab program which utilizes an interdisciplinary approach consisting of: 

• Physician
• Physiatrist (Doctor of Physical Medicine)
• ENT Specialist
• Psychologist
• Physical Therapist, Occupational Therapist,
• Speech Therapist
• Respiratory Therapist
• Recreation Therapist
• Registered Dietician

Nursing Rehabilitation is coordinated by a Registered Nurse who is a Certified Rehabilitation Nurse - CRRN

Our program includes:

• Comprehensive Care Planning to meet the individual patient needs and goals
• Patient/Family Education
• Psychological/Emotional support
• Complete Nursing Care
• Periodic progress reports to private community-based physicians
• Discharge Planning, including home care equipment and services
• Post discharge appointments with primary care physicians and sub-specialists
